There is no need for me to have another medical exam because i am done with that when i was still in my country. I just need an additional vaccine and it should be done by USCIS designated civil surgeon.

Yes you are correct, but you need to have your DS2035 transcribed to the I693, correct? If you take the DS2035 and your passport to the place I listed above they will do it for free, or you can send it off to Dr Arnold and pay 30 bucks. Up to you.

Then peeps should go for it if they live in the area. I doubt if they do it by mail as DR. Arnold does. I had friends here in Hawaii use Dr. Arnold with success. I wish I'd known about him as we had to fly to another island to see a DCS.

We called them and it's not for free. You need to pay $100 just for them to transcribe the vaccination supplement and they don't perform vaccination also. They will send you to the health department to do the shots. Seems costly and a lot of work...

Do you already have the DS2035 filled out from when you got the medical exam in your country? Maybe you need to explain exactly what you need to get done. They did mine for free, I explained all I needed was the information from one form put onto the other form and sealed up.
