DCF - Delhi - Address to use?

Hi,

I will meet my 90 Day DCF requirement soon for Delhi Embassy. I am filling out my I130 and G325A forms and confused on which address I should use. The Embassy mailed me and said their residency requirement is 90 continuous days.

Can I use my US Address for filling out all forms (as after submission, I plan on returning to the US, for business reasons, etc and will be there for few months)?

Also, where it says residence for last 5 years, must I put my stay here in India (since it was only 3 months and stayed with my uncle). I only put my US locations in last 5 years.

Please help with guidance.
