I used the cover letter from the wiki short cut...is my packet complete? I had to squish everything so it fit on one page and I put the barcode in the top right corner

• Two (2) Passport Size Photographs of Beneficiary (NVC Case Number, Beneficiary’s full name, and date of birth are

written on back of both)

• DS-230 Part 1 completed and signed by Beneficiary

• DS-230 Part 2 completed by beneficiary – unsigned, as of yet, per instructions

• Cover letter of IV fee bill PAID

• Photocopy of the Biographic Data Page of Beneficiary’s Passport

• Birth Certificate of Beneficiary

• Certified Translation of Beneficiary’s birth certificate

• Marriage Certificate

• Certified translation of marriage certificate

• Original Police Certificate(s) for Beneficiary

• Certified translation(s) of police certificate(s) for Beneficiary
