I submitted an affidavit of birth for my husband because his birth was never recorded. My mother in law doesnt speak english, so, we got the affidavit of birth from his eldest sister. The affidavit must be notarized or certified or it will be rejected by NVC. We did'nt have any problems with our submission. Hope this helps! :reading:

Yes, I only submitted one, from his eldest sister. It must include names of both parents, their dates of birth and deaths, where your mom was born, and the date and year of her birth, and how whoever is submitting the statement has knowledge of these details.
