The latest version of the I-134 does not need to be notarized. You apparently completed the outdated form? Hopefully it won't matter that its not notarized.

If the old form was used, it needs to be notarized. The language has changed in the new form so that the signature is under penalty of perjury. The old for doesn't have that language and MUST be notarized.

No, I used the lastes form from. It has a expired date of 5/1/2010
