Hello All,

I just wanted to know if we all have to sumbit the additional questionnaire at the time of interview or it is only when one is put on AP?

HSB,

Did they send u Add. Ques. form with the interview packet?

Members who have experienced this before, plz share this information..

Hi Sonia,

The NVC sent my interview letter, and it just contained the date and a checklist of all the documents they received. It advised to complete the medical before interview, but it did not really provide instructions on how to do so, I used the instructions from the embassy website. It did not mention anything about the additional questionnaire document, only the embassy website seems to mention it.

I'm not sure if I am supposed to get another interview letter from DOS or embassy which may mention the additional questionnaire form, but I received nothing else so far other than the NVC appointment letter. I am still planning on filling out the additional questionnaire document and bringing it with us to the interview just in case, but my best guess is it probably will not be needed unless placed on AP. Alot of the info on the Dhaka Embassy website about IV visas seems to be outdated.

I know Shimool's wife had her interview last month so he may be better advise whether the additional questionnaire was collected, but have not seen him on VJ in awhile.
