My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

You have to have a medical emergency or have to be deployed in the Army or other services to have an expedite request. What I did for mines was I called NVC and told them my medical emergency. The operator gave me her supervisors name that I should email explaining in detail what the medical emergency is and attach all proofs including a doctors note along with the email. After that they forward it to the US consulate in the beneficiaries country and they wait for them to reply back with an approval or not.

That's so hard to do, I even tried that when there was bombings and street fighting but nothing worked...

The National Visa Center is just who you send the expedite request to... they don't make the decision. You don't need to have a medical emergency or military deployment in the present or future to request an expedite, you can request one for any reason... that's what a supervisor at the NVC told me. It's up the the Embassy or Consulate that will eventually receive the case whether they will expedite it or not. Sending in the request apparently doesn't effect your case, even if the request gets denied.
