Form I-864A Question

Hi,

I am sponsoring my mom and using my husbands income in addtion to mine. He needs to complete I-864A, bellow I attached the form I-864A.

On question 13 I the sponsor (this should be me, right???),............

There is something I need to complete and under it says (indicate number).

My question is:

1) Do I to put 1 since I am sponsoring only my mom or what else?

2) After that do I need to put her name and date of birth.

3) On question 15 I put my husbands name and again under indicate number we put 1

Please correct me
