i called one office in mt vernon to ask for an appointment and the earliest date they could accomodate me is on the may 20th! but i set the appointment anyway and then they told that it'll cost me $500, the least, for the medical examination. so i went on to tell them that i don't need the full exam, just for my vaccination record to be transcribed. but they told me that i still have to do the exam all over again and even if i opt to just get the vaccinations, i'll still pay $500. and it's because i "need the doctor's signature"! those were their words! damn that signature!

i called up other offices, and most of them answered that there were no civil surgeons in their office. but they were listed at the uscis website! so i figured it could be that they have clueless secretaries/receptionists and i might just have to storm into their offices and read to them the instructions. and i did. at the last office i went, they knew immediately what i needed. it'll cost me $100 to transcribe my vaccinations, still a bit expensive for just a signature but way cheaper than the others. plus, they told me i could get the tb skin test at the naval hospital, where i could get it for free. they could do the test for $23 but that could be gas money for me already. so now, i just have to call them up again for an appointment and bring the results.

anyway, i think these doctors are too abusive of their position!

You can have your vaccination supplement completed by Dr. L. E. Arnold of Austin, TX for a minimal fee. Google his name and contact his office and they will work with you over the phone/mail. I paid $20 6 months ago.

I for one was tired dealing with the civil surgeon to transcribed my medical exam, they told me to get this shot and blah blah which ofcourse they are really expensive so I took the risk by not submitting the sealed envelope for the vaccination and Thanks God, i now got my green card without having RFE for that I-693 form.

$100 is still too much. If after reading them the instructions they still refuse to do it for a nominal charge, you should just notify that you are reporting them to USCIS and go to a different doctor. These surely aren't the only ones in your city, are they?

I went through these same dance a little while back. I'll find the post I wrote about it.

i've read about him too. however, people said he doesn't transcribe the tb test, just the vaccination record. any truth to that?

actually, i've already sent my aos papers. but i just want to be prepared just in case they ask for it during the interview or if they send me an rfe.

I'm trying to look up other cities. mt vernon was the nearest. maybe i could still get it cheaper, considering the gas i'll use up.

I had the same situation, I called Dr. Saleem Memon's office, who is the designated surgeon in my area and they answered that the doctor will not fill in any paper without physical, and that would cost 250$ for one person, so 500$ for me and my child. And they even can not administer missing vaccines. But I tried another surgeon, the closest one, which is about 4 hours of driving away, and they promised to complete the vaccination supplement without physical for 45$ a person, but they also can not administer any vaccines
