Timeline

i am looking at proccessing times here, what happens after my wife's visa expires until she gets her green card? it looks like it could be a year before she gets it and her visa expires next month. we are hoping to submit the AOS paperwork this weekend. also does anyone know if she will have to have another medical exam? she had one in march for her visa but i think that is only good for 1 year, what if her green card is delayed longer than that?

As soon as you file your AOS your wife's status changes to Adjustment pending... so it does not matter that her K1 has expired...

She will not need to have another medical as long as she has had one done within 1 year of filing for AOS... make sure you send the vaccination suppliment with your AOS... you can get that done by taking her vaccination record to a civil surgeon and they will copy the information onto the form and seal it in an envilope for you to send with your AOS application...
