On the embassy list of requirements, it says for the visa fee, "The bank will validate your payment on the DS-156 Nonimmigrant Visa Application Form or on the DS-157 Supplemental Nonimmigrant Visa Application Form." So my fiancee needs to bring her completed DS-156 or DS-157 to the bank when she pays the fee? She hasn't filled these forms out yet because she's still waiting for delivery of her passport. Thanks.

OOps Sorry I didn't notice you posted in the PI section.

Have her take 2 copies of each. Normally the bank will give her a stamped DS 157 and she'll have to fill it in by hand and make a copy.

No. She just go to BDO (Bangko De Oro) and tell the bank personel that she is buying US visa form. Pay by philippine peso. Verify if there is a BDO near her area that sells US visa form. Check the Banco De Oro website.

Does the fiance sign it before she makes a copy or do all signatures have to be original?

The DS 157 form from the bank will have a stamp on the back indicating payment of the visa fee. The form is not filled out. Your fiancee can fill out two DS forms later downloaded from the USEM website and staple the stamped form to one of the two. Or can fill out the stamped DS form and fill out another. Two signed original forms are required for submissin to USEM. You should keep a copy (xerox) for yourself.

Thanks everyone for your input. We're sort of in a holding pattern because of her passport, but I'd still like to take care of some things while we're waiting. I'll have her try to pay the fee next week. Thanks again.

If you are submitting those papers to the embassy, make all the signature original
