Well, a CS did sign off your DS-3025 - it was just a UK based one is all. As for whether or not you send an I-693, the question I would be asking myself is "why not?" as opposed to "why?" Sure, some people have submitted just the DS-3025 and been aprroved just fine, but some people have been RFe'd for it. And if it's RFE's that you're keen to avoid, then there's only one option for my money - send in an I-693. Dr Arnold's service is a measly $30 - it's a given in my book.

I seriously don't know one couple that was RFE only because they didn't use the I-693 form when they had a totally complete DS3025. Because we were in the transition period and the guinea pigs for what would work after the new changes, I have read and studied a gazillion threads and posts on the matter every day since October. If you really dig and read and follow up on the people, the posts that said "we got a RFE for the medical" are because the medical was lost between POE brown envelope and AOS adjudication. That happens alot and they have to do the whole medical over because there is no record of their bloodwork, chest x-ray, mental health, etc. That sucks. But the RFE isn't actually because USCIS wanted the immunizations on one form and not the other. There were always other issues that caused the RFE. So in my studies of this issue, I have found maybe 20 now that sent a complete DS3025 and got a green card. I say SK Parker---Just Do It.

And Parker, you don't need all those cover sheets. You mail to Lockbox and they do the sorting and sending elsewhere. They probably toss those cover sheets in the bin in the mailroom. One cover sheet similar to the one I posted on p.6 of this thread that lists the submission should be plenty good. My letter starts with:

Well, I am not one of you all UK AOS'ers yet...:no: but i had a q about transcribing the DS3025 onto I-693 by Dr. Arnold's services.

My fiance did not ask for any vaccinations at Knightsbridge and neither did he get them done with his GP because his GP was a pain in the ####### to deal with...so we decided to leave them for AOS here in the USA. So I know that his Knightsbridge medical is valid for 12 months from the date of the medical, but is it possible to get Dr. Arnold to transcribe the DS3025 AS WELL AS records of his required vaccinations done by a primary physician here in the USA(not a CS) both onto the I-693? Or do the vaccinations have to already be on the DS3025 from the Knightsbridge medical in order for Dr. Arnold to transcribe it all on the I-693?

As long as you send a letter from the doctor who administrated the vaccines in the USA for you to dr arnold, as well as the ds3025, they will transcribe it. I decided to have the HPV vaccine here in the US and they transcribed with no questions and my case has been approved.

Arent you supposed to get all the vaccinations done at the medical before entering the country??? I thought that's why i read so much about how people have to wait to get their visa because they are waiting on the medical.

Are there more shots that need to be done that I havent read about? because everything that was on the list that needed to get done for the medical at knightsbridge was done. hmm :blink:

Completed immunizations are not required for the visa. The list is American and some shots are probably not even available worldwide. I think the HPV might be one of those that many girls get once they arrive in the US. For them, yes the I-693 has to be filled out to show the shots received in the US. If your sheet is done (probably needed Tdap and MMR for a guy) then I think you're good to go. Just hope your medical report in the brown envelope meets up with your AOS applications. Have a look at our lUK AOS timeline and you'll see the following who got approved for AOS without an I-693:

Nich-Nick, Roselinda, Talyss, and TinkerbellUK. I know of others from Belgium, Italy, Canada...can't think of where the others are from. I did have a list going but can't find it. Most were transferred to California and were approved there without an interview. But one of my friends from Italy interviewed at a local office last week and was approved on the spot without any question of her medical form.

Rocks you have your AP in hand? I think you would have to make an Infopass appointment. I would definitely not travel using it . (which sounds like you wouldnt atleast before getting it changed. The only problem is since it was your mistake (and not USCIS) I wonder if you would have to pay another fee.

On another note, I have been trying to add my I 485 to my USCIS account alongside my I129f but I cant find anywhere that allws me to add the case to my portfolio, hmmm.

yes i have the ap and i obviously won't travel on it. it wasn't my mistake at all, it was 100% their mistake!

my first and last name are correct but my first middle name isn't there, it has my second middle name as my first and my maiden name as my second middle name. for example, say my old name was sally anne jane andrews and my new name was sally anne jane matthews, it says sally jane andrews matthews. i have no clue how that happened but this definitley was not my fault!

infopass is the only way?

i had a problem with adding all my cases to my portfolio, suddenly a button below appeared that was not there before, not sure how that happened.

i just looked at his records and he DID get Tdap....his family gave it to him beginning of 09 and the doctor listed off ALL the vaccinations he had--which was EVERYTHING he needed. but on the ds-3025...only TD is marked off, not TDap....#######. is this their error?? should i contact anyone?? i dont want to get a RFE....so i figure maybe i can send the letter from the doctor just in case (since i already sent aos off)

I only had TD marked off not Tdap and it went through ok. As long as the doctor ticked the box to say he is suitable for a blanket waiver it should go through ok. The differences between different immunisations and who should have what at what age is confusing enough for most health professionals, I am sure USCIS officers are going to be looking to make sure the waiver box is ticked and the form signed off correctly rather than be trying to interpret for themselves if the person has had what they should of had.

I am able to see my EAD and AP case online for the first time today, seems they sent them out to me yesterday. You think the computer system would flag that they are no longer needed to save on some Admin costs.

Td is fine. Tdap (drug name Adacel?) I I think is only specifically available in the US. The p in UK childhood immunizations includes polio but in America DTP, dTap and Tdap, the P is for Pertussis (whooping cough.) And I think the UK doc checks off you've had the P thing as a baby and nobody is aware of slight descrepancies...especially not the USCIS adjudicators. Anyway, forget the technicalities. They will approve if Td is checked, which is basically a booster of your baby shots and is good enough.

DTaP and Tdap (where the p stands for pertussis) are a routine part of the immunisation programme for children in the UK but are not usually given as a routine booster to adults. The routine adult booster is Td/IPV so I think the embassy doctors sign it off as not age appropriate.
