I notice you're local office is in Columbus, Ohio! Are you from there? my wife is going to be taking her driving test here in the near future!

We're in Dublin but I took my test in Hilliard, it was a doddle...She should really pass no prob!

Well, I am not one of you all UK AOS'ers yet...:no: but i had a q about transcribing the DS3025 onto I-693 by Dr. Arnold's services.

My fiance did not ask for any vaccinations at Knightsbridge and neither did he get them done with his GP because his GP was a pain in the ####### to deal with...so we decided to leave them for AOS here in the USA. So I know that his Knightsbridge medical is valid for 12 months from the date of the medical, but is it possible to get Dr. Arnold to transcribe the DS3025 AS WELL AS records of his required vaccinations done by a primary physician here in the USA(not a CS) both onto the I-693? Or do the vaccinations have to already be on the DS3025 from the Knightsbridge medical in order for Dr. Arnold to transcribe it all on the I-693?

Marm, I needed extra shots that I wasn't prepared to pay Knightsbridge 60 notes for. I had them done at my local GP, so obviously they weren't on the DS3025. I sent both the DS3025 & my vaccination record from my local GP's surgery to Dr Arnold, and both were transcribed on to the I-693 with no issues at all.

I also had shots in the USA (HPV to be exact) , just make sure you ask for written proof that you got them, the nurse that gave me mine wasn't going to give me anything and I had to ask for written confirmation I got it.

That's awesome. I work in Dublin over by Tuttle Crossing, and live down in Grove City. She was laughing at the driving book and tests...She just wants to get a few hours driving in as she's never held a license before, so we're starting from scratch. I told her the same thing though, the driving test is easy as pie.

I've been driving for 10 years and still can't park and the guy actually directed me through the cones so I didn't hit any!

Thanks everyone for your replies...that is one load off my mind. I am preparing a "TO DO" list for AOS in chronological order so I am not stuck with last minute surprises, can't afford those any more!

My husband only has the 3025 vacination sheet--he had all of his shots done at a regular doctor so just had to have x-rays n that stuff done at knights...

the local doctor sent all the records to the medical and they had the 3025 sheet written up with a signature at the bottom...

is it okay we didnt have a CS do any of this and we are just sending the 3025 form...

we really dont want any RFE's.

another question....

i have made 4 coversheets....one coversheet that says what is included (i-485,i-765- and i-131) and then i made coversheets for each of those.

i know the i-131 has the same address (to the chicago lockbox) as the i-485...i know they all get sent together. im trying to put addresses on all the coversheets but i know that the i-765 has a different address---i believe the nebraska service center since i am in illinois.

i will be sending all 3 of these to the chicago lock box...but should i address the 3rd one to nebraska (im not sure if they forward that on for me).

thanks for any replys and real sorry if this question has already been asked. :whistle:

Well, a CS did sign off your DS-3025 - it was just a UK based one is all. As for whether or not you send an I-693, the question I would be asking myself is "why not?" as opposed to "why?" Sure, some people have submitted just the DS-3025 and been aprroved just fine, but some people have been RFe'd for it. And if it's RFE's that you're keen to avoid, then there's only one option for my money - send in an I-693. Dr Arnold's service is a measly $30 - it's a given in my book.

I seriously don't know one couple that was RFE only because they didn't use the I-693 form when they had a totally complete DS3025. Because we were in the transition period and the guinea pigs for what would work after the new changes, I have read and studied a gazillion threads and posts on the matter every day since October. If you really dig and read and follow up on the people, the posts that said "we got a RFE for the medical" are because the medical was lost between POE brown envelope and AOS adjudication. That happens alot and they have to do the whole medical over because there is no record of their bloodwork, chest x-ray, mental health, etc. That sucks. But the RFE isn't actually because USCIS wanted the immunizations on one form and not the other. There were always other issues that caused the RFE. So in my studies of this issue, I have found maybe 20 now that sent a complete DS3025 and got a green card. I say SK Parker---Just Do It.

And Parker, you don't need all those cover sheets. You mail to Lockbox and they do the sorting and sending elsewhere. They probably toss those cover sheets in the bin in the mailroom. One cover sheet similar to the one I posted on p.6 of this thread that lists the submission should be plenty good. My letter starts with:

As long as you send a letter from the doctor who administrated the vaccines in the USA for you to dr arnold, as well as the ds3025, they will transcribe it. I decided to have the HPV vaccine here in the US and they transcribed with no questions and my case has been approved.

Civil Surgeons are a pain.

We went to one, and it was a long 2 hour process. They made a fuss over Sarah's papers as she had to wave getting several shots due to being pregnant until after the baby's born. They acted as though it was a huge hardship. just my two cents :P
