I CANNOT FIND THE FORM I-693A!!!!

Please please help. i am stressed out of my mind right now. i need to download and print the i-693a form. it's about supplementing vaccinations or some #######. i don't even care anymore. i can find the I-693 form but i need the 693a. please help. send me a link or something. thanks

i-693a is a form used by civil surgeons - not completed by us. (That's why you can not find it.)

It no longer exists. It was incorporated into the I-693 last year (as Part 2 of the form).

The vaccination supplement element only needs to be transcribed onto the I-693, page 3 .... only pages 1, 3 and 5 need to be completed on this form AT ALL, provided the original medical was signed off.

Don't let the Civil Surgeon bully you into taking the full exam again.
