My mother was married before she married my dad - do I use her name on my birth certificate ( ex married name ) or her real maiden name before any marriages ?

Her Maiden name , the name she had when you were born to her at birth . It would be on your birth certificate , under your mothers information . Just dont forget to include a photo copy of your birth certificate ( front and back ). The back side so they can see the seals on your birth certificate , and send the G-325A Biographical Information that includes your fiancee information on the 4 pages . And have her sign and date them , with the current date , and have her send you a copy of her birth certificate too ( front and back ). She can do this the same time , she sends you the signed G-325A Biographic Information . To you in postal mail

example: her birth name is jones

she married and become smith

then divorced

her name on my birth certificate is smith

but her real maiden name is jones

so do i use jones or smith ?

When asked for your mother's name, use her current legal name. If asked for your mother's maiden name, use her maiden name. The G325a asks for her maiden name. That would be the name she had the day before her first marriage. Read carefully and interpret literally. If you don't understand a term like "maiden name" google is your friend.
