That is an extremely basic question clearly specified at:

Scroll down to the N-400 link, open that and make sure you download that 68 page M-476 manual, the instructions, and the form, and read and study the entire M-476 manual plus the instructions, both work hand in hand.

One of the civic questions on my wife's old test, is which form did you send to apply for USC, but just knowing it's the N-400 is not enough, her IO went through that form with her and asked her questions about it, and you have to be able to explain your answers, so you have to know that form very well, can't just hire an immigration attorney and have him plug in the answers.
