Timeline

The instructions for the 129F form ask for a copy front and back of my birth certificate. When applying for my passport last year I ordered an official copy of my birth certificate. It is on official paper, embossed and stamped and was official enough to receive my passport. Do I make a copy of this document, which doesn't really have a back, do I have to send this official copy in, or am I looking for something different?

They also request a birth certificate number and date issued. There are several numbers to choose from. Are they asking for the Child's birth number? What about the date issued is it the issue date in 2008 or when I was born?

I am referring to a State of Illinois birth certificate.

Are you using your birth certificate to prove your an USC?

Your passport is sufficient. (full copy of it that is)

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

If you're referring to Section 10 - they're not asking for Birth Certificate number. Am unaware of anywhere asking for BC number...

What is "number of certificate"

Section 10 is referring to your citizenship.

Check box for "By Birth"

or

Check Box for "By Naturalization"

If you check "By Naturalization" you must enter your naturalization certificate number. Otherwise leave this part blank if you selected "By Birth."

I had the same question about my birth certificate. You have the one you need just make a copy of the front and you'll be fine. I attached another sheet of paper saying that there was no back to my BC and signed and dated it but I dont think you need to go that far. I was just nervous :wacko: To you other question the other posters are telling you right. Go to the guide section and look at the example form of the I-129 and you'll see how to answer that one. I found the example form really helpfull to answer most of the questions I had with the I-129. Good Luck to you! And just take your time and you'll be fine.

The Illinois birth certificate has water marks on the back. They need a legible photocopy. Adjust your copier darkness and you will pick up the watermarks on the back, Unless you got a photocopy of an older microfilm document, in which case, copy the "blank" back and write "copy of back of birth certificate" Depending on how old you are, some of the records are archived and you get an official, stamped document which replaces the old type birth certificates.

Basically, copy front and back of what they gave you, wheter it has anything on the back or not. It sounds like you have the newer birth document. It was not "issued" the day you were born, so use the date it was "issued" to you. The childs birth number is the "official" document record number, use that.
