do i need to change my name on my passport and also the address that i wrote inside now that i have a greencard?

do i have to send in seperate forms to change name and change address? and des that mean i need to pay both fees?

No, you don't have to change the name on your passport unless you want to. I'm assuming the address is there so you can be contacted should your passport be lost and then found. So I guess if you'd like it returned then it's probably best to insert a new address in there. You can probably find the answers to the rest of your questions at the Passport Canada site.

Hi Orange_n_pink,

No, like Krikit said in the post above, you don't need to change your name on your passport unless you want to (for example, for a maiden last name to a married last name). If you want to change your name on your passport, then you would have to apply for a new passport all over again.

As for the address, you can do that maunally on page 4 of your passport (on the "Loss or Theft of Passport page). It gives you space to write your name and address with pen, and it says the following: "PLEASE FILL IN PARTICULARS BELOW AND AMEND AS REQUIRED". Just cross out your old address and write your new one above and/or below it. As well, you may also wish to notify Passport Canada of that change, to make it "official". I think this address change only officially with Passport Canada can be done for free, but you will have to inquire further with Passport Canada about that.

For both name change and the address change, that can be done at the same time when you apply for a new passport. The fee that you pay is for the new passport, which is about $90 CDN.
