Did you send a copy of the 1 year extention letter? That could be it.

I had the same issue. My wife's application package was returned to us including the check. The NOA came with a list of 6-7 things that needed to be corrected or added to the application. At least half of them made no sense at all as the information already there was correct such as my wife's name and the need to include photos that we already included. We responded to the remainder of the items and sent the application back to Texas. The new check was cashed yesterday so hopefully this means that the application has cleared the documentary hurdle. I am under the impression that USCIS computer that generates the NOA spits out a block of problems that statistically correlate the to real problems that need to be fixed and does not worry if all of them apply.
