Hello again, I'm kind of confused today because my hubby called me and told me to download the Biographical form and fill it up again...huhuhu He told me that we commited mistake when I wrote my last name when I was single, supposedly I should have written my hubby's last name in that form. I'm kind of undecided on what to write in my DS-3032 (Choice of Agent), what will I write? Is it my last name that I used in the biographical form or the last name of my hubby? My hubby made a letter already showing that we committed mistakes in our biographical form, and will be sent on Monday to NVC. Is this advisable? Oh my gosh, I'm really worried about it. I don't know what to do....Pls do help me...

Before processing our papers, I changed the status of my passport already, I used my hubbby's last name in it. Is this ok?

Changing your passport in your hubbys last name is ok, it is you should do, since you are married already and for the ds3032, i suggest to make your husband as your agent, write your husbands name , his address ,contact number and email add.. you can look at this shourtcut if you like

maybe your hubby need to call the NVC as well to know the response asap, writing them email and letter will took so long , for email it will took 10 days before they response, this is only a suggestion, i know some here in VJ encounter same problem that you have...goddluck :) ..dont worry to much,everything will be fine

Yes, basically, I'll choose my hubby as my agent by writing the complete info about him. The thing there is, I don't know what to use and what will i write, is it my last name or my hubby's last name? When i did fill up the biographic form, I used my last name not my hubby's last name. Shall I write the same last name just like what i've done in my bigraphic form?

I think first thing you do is to have your hubby call to NVC and see what they recomend or i think you should use your married name coz when you have to file next form which is the Ds 230 they will ask your biographic passport page ,which has your married name on it, since you said that you change it already to your married name. But still i suggest to call NVC ....they may correct it for you i guess...

Hello it's me again,

Pls advice us...Both us are really worried and haven't sleep well due to these reasons...

Are you from the Philippines? If you are married - PLEASE CHANGE YOUR STATUS AS MARRIED INCLUDING UR NAME.. UR NAME SHOULD NO LONGER BE THE SINGLE NAME YOU HAVE BEFORE..

You might face some problem in NVC And USEM and the consul might ask you how come you are married to your husband but you are not using his name.

Better change all your documents with your MARRIED NAME

Simply means RequestForEvidence RFE. Whenever NVC sees an incomplete application or if they need something, they will contact you and hold off on the processing. Just delays the process.

Don't worry, many people get it but stay on top of things. Good luck!

hello everyone. hope you guys can advise me what to do. i already fill up the form d230 part 1 only and didnt fill up the part 2 because i thought you just need to fill it up when the interview comes. i have send it directly together with required documents. do i need to send them back another form d230 with part 1 and part 2 completely filled up with part 1 with signature? or do i have to wait for their response before sending?

You fill out and sign ds230 Part I

You fill out only ds230 Part II NOT SIGNED. The beneficiary signs it in the interview.

Please fill out the DS-230 Part I and Part II completely. If a question does not apply to the applicant then the applicant must mark it with “N/A”.

Note: The applicant should sign Part I only at this time. Part II of the form must be signed at the time of the applicant’s visa interview.

Please resend them the papers since you will get RFE for incomplete Part II.

I've been married since 5/24/08 and I didn't change my name after I got married and not planning to do so. One would think you would only use your husband's last name on your filed documents if you plan to take it and are changing it legally. If not, there should not be a question as whether to use your maiden name or married name.

Hello to everyone. :D I'd like to ask regarding DS-3032, my hubby just received the AOS Bill and a copy of Form DS-3032 yesterday (March 29,2009). Ahm, will I be receiveing a copy of Form DS-3032 from NVC at the same time?Who's gonna fill it up, is it me or my hubby? Pls advice. Thanks.

Your petitioner can fill ip up but the benificiary still need to make the petitioner if they choose to as the AGENT... the DS230 will be the benificiary as she need to signed it.

Hello...i am new user of this website...Actually i wanted a help, if anyone can tell me where can i get the D-230 form...I searched through the net and I found MS word form which seems to be informative only. Please let me know the link from where i can copy the form...
