DS-3025 vs I-693

Hello;

my wife(K-1 visa holder) have completed her medical prior to the interview at the Moscow embassy and received a Form DS-3025 less than a year ago. She successfully made it to US, we got married. We are just about to send the all doc to adjust her status. I was going to include the Form DS-3025 as part of the required documents as it is suggested in the Captain Ewok’s post. BUT, I noticed (for the first time, never even looked at the form before) that at the bottom of the form below the list of immunizations, the “Incomplete immunization, one or more shots required” box is checked!

Could anybody advise me on the course of action? Should I just bring this form to the civil surgeant and prepare a form I-693- part 1, part 2-vaccination section, and part 5 in the application? Or, I just need send DS-3025 as I have it now?

You will need to get an I-693 (Vaccination record) don by a US Civil Surgeon.

K-Visas are NON-Immigrant so do not require vaccinations until they adjust status to IMMIGRANT.

My wife got vaccination after visa interview and before leaving China, we then had that record transferred to an I-693 in the USA.

Weird, I had my timeline completed long time ago, not sure why it is empty right now, will do it again tonight.

you do need to visit a civil surgeon to have the immunization series completed. check around as some offices are incompetent and prices can vary immensely. you can send your AOS application without the completed I693, but you will need to bring that with you to your interview (in a sealed envelope from the civil surgeon).

thank you. I think I'd better send the copy now to avoid RFE as I read in some posts here. But my wife will definitely have it with her to the interview.
