On my LAST entry I entered with an H1B, so should I say I entered as an "H1B Employee"?

Also on my I-94 there is a handwritten date of Sep 2009. Is that the expiration date of I-94? What does it mean?

Yes you can write it down as "H1B Employee".

September 2009 is the expiration date of your admission.

In other words, your H1B visa will be expired on 09/2009.
