Can somebody provide me with the format for a coverletter? I will be filing I-130 & I-485 applications concurrently, adjusting status from H1B. Also, do I have to attach an additional letter for documents that serve as evidence of bona fide marriage, like bank statement/car insurance? Thanks for your help!

Check the guides above, you can use the one for the K-1 process for the cover letter.

Just change the wording for your case :)

No, just list the documents on you cover letter.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.
