Vaccination record requirement

Hi all, Vika asked me to post this up out of nervousness. She has a vaccination documentation worksheet (in English), officially stamped, from the clinic in Kiev where she had her medical exam. SHe wonders in the worksheet alone is sufficient to get her past the filing requirement. She read on another site that she also needs to see a doctor here to put the vaccination records in some other official form. Thanks for the help!

I also have the same vaccination record during my medical but I still went to a Civil Surgeon ( Accredited by USCIS) for my form I-693 that only a civil surgeon can do.

February 24, 2008 - He came to the Philippines to finally meet me!

February 26, 2008 - He proposed to me and I said YES!

March 4, 2008 - Flew back home ( US )- sad

K1 timeline:

April 7, 2008 - sent I-129f to VSC

April 10, 2008 - VSC received petition

April 18, 2008 - NOA1 hard copy received

May 9, 2008 - touched

July 14, 2008 - touched

July 15, 2008 - NOA2 (99days)

Aug.14-15,2008-Medical PASSED

Aug.22,2008- Interview PASSED (Pink Slip Only)

Aug.29,2008- VISA ON HAND

Sept.3,2008- POE Houston TX

October 25, 2008 - Officially Mrs. Eaton

AOS Timeline:

March 10, 2009 - AOS package sent to USCIS via FedEx

March 12, 2009 12:21pm - AOS package delivered to USCIS

March 20, 2009 - Check cashed

March 21, 2009 - NOA1 for I-485, I-131 and I-765

March 24, 2009 - Received Biometrics Appointment

April 4, 2009 - Case transferred to CSC

April 7, 2009 - Biometrics appointment is done

May 4, 2009 - AP approval notice received

May 8,2008 - EAD received

June 3, 2009 - Welcome notice from CRIS via email

June 8, 2009 - Receive hard copy of welcome notice on the mail

June 15, 2009 - Card production ordered

June 27, 2009 - Green Card on hand ( 2 years )

And I forgot to mention two very important things:

1) - Always download the latest application forms.

http://www.uscis.gov/portal/site/uscis

Click "Immigration Forms" in the top banner.

2) - Always read the instructions for each one of these forms.

They will explain you what to do.

If you have a question or a doubt, the forum members are here to help. You can do a search first before creating a new topic, though I must admit the search engine is definitely not "Google" :)

The DS-3025 is what we have.

All I can say Brad is it worked for us and it has worked for others.... Yes, the forms changed last May... If it were me, I would submit only the DS-3025.... Since vaccinations are not required to be submitted with the original submission (can be presented at the interview). If you have the luxury of an interview, have it translated by a CS at that time....

I had the same problem. I had a vaccination record from the American Clinic in Moscow where I had my medical. It was in English. But when I presented it at AOS interview they said that I had to go to a Civil Surgeon as it needs to be a special form. So we had to go to one of them and they just transferred my vaccination to a special form. Just do it before the interview, it only takes 10 minutes.

If you decide not to transfer to the 693 you are taking a risk that you will received an RFE but it can be a calculated risk.

I had my medical done in april 2008, and received DS3025

I sent that in with my AOS in September 2008 and received GC with no RFE in March 2009.

K-1 Application

10th October 2007 - NOA1 issued

31st January 2008 - NOA2 issued

22nd May 2008 - Interview date

18th July 2008 - US here I come

22nd August 2008 - Wedding date

15th September 2008 - AOS Pack (I485, I131, I765) Sent

22nd September 2008 - NOA1 x 3 Received

6th October 2008 - Transferred to CSC

17th October 2008 - Biometrics

5th December 2008 - EAD & AP approved

20th February 2008 - GC approved

I-751

================

1st December 2010 - I-751 Sent (CSC)

3rd December 2010 - I-751 Received

7th December 2010 - Cheque cashed

3rd January 2011 - Biometrics

So, you didn't go to a CS and such? WOW! I am starting to think on trying that.

Mar 12 - 10 yr GC received

Aug 13 - Naturalized
