I have my AOS interview In April, I'm a little worried about the medical exam, so before I came to the US I had a medical exam with a civil surgeon who works with US Consulat. and this was in Jan 2008. I came to the Us in April 2008. and the medical exam was in the sealed envlope we give to the customs. when I filed My AOS in August 2008 I only gave the Vaccination record (small card) that I had in France with the doctor; they say with K3 visa we don't have to do it again. So now I have my interview in April 2009. Do I have to submit The I 693?? Or will only submitting my vaccination record be fine for the interview. I appreciate your help.

If you medical exam was less than a year old at the time of filing the I-485... then a new exam is not needed.

YMMV

I have similar situation, dami...I had medical in March 2008, AOS application in July 2008, less then a year. I did not submit I693, but submitted only copy of DS-3025 listing all shots, done by Russian doctor (although it is in English). IDd not get any RFE's on that. Now - interview is in April. Will they ask for I693 then? I know that Dr Arlnold - texas, could transfer DS-3025 to I693, but do I need that, since they already looked at my papers and did not asked for it?

You cannot be expected to be prepared to provide something if they did not ask for it....

YMMV

Hey dami,

I'm in the same situation but I got the DS-3025 from my doctor in Paris. It is the vaccination documentation worksheet. I also have my chest picture and my blood test results. Do you have this documents also ?

I will bring these documents with me for my appointment with a civil surgeon here in the US. You only need with these document to get a supplement of medical and not the full medical exam. It is $50 instead of $300 for the complete one.

I have a question, where can I found the I-693A form ? I only found the I-693 which is for the complete exam. Do I have to print the I-963A or maybe the civil surgeaon has these forms at his/her office ?

Thank you

There is no form I-693A anymore. You have to use I-693. You fill out part 1 and the Civil Surgeon fills out part 2 (vaccinations only) and signs part 5. The parts about the medical exam can be left blank. It says so in the form I-693 instructions.
