Well you won't need your visa now that you have your green card. Your visa will be null and void anyway. But you will need your passport. How bad of shape is it in? You may need to apply for a new one ASAP.

My passport is damage also while i am waiting my GC, now i am worried if we got a interview for AOS and my passport is damage. What if the consular ask my original passport what i am going to do?

I am very worried and stressful, hopefully someone who can help me!

Do i need to apply for Passport Amendment or Report Marriage so i can get a new passport?

Same thing happened to me I used my passport even to travel back to Philippines and to Mexico once - no problem, until it got worn off and the poor thing suffered further damages. I traveled to Mexico the second time I got shipped back to US due to severe damage, the outside cover completely came off if I had pasted it together I would have been fine. But I will not suggest traveling with it outside the US - too costly for one, I put off traveling until I got my US passport. Renewing my Philippine passport would have cost me twice the amount of when I acquired my citizenship and passport altogether. Just a piece of advice.
