Return contents of my Packet 3

Hi, i'm terrified that I am going to mess up the packet 3 stuff like I did the I-129f! Please can you check the list below if you have time!! to see if what I have listed to send back to the embassy is correct. Also, I am sending copies of forms for my 11 year old...not sure if I need to fill all of them out for her:

1 copy only of IV-15 checklist but with my daughter named on applicants statement.

1 copy of DS-230 part 1 for myself, and 1 copy for my daughter

1 copy of DS-157 for me and 1 copy for my daughter

1 copy of DS-156K for me and 1 copy for my daughter

2 copies of DS-156 for me and 2 copies for my daughter

Additional sheet with continued employment history and trips to US listed.

I do not sign ANY of these forms, correct? I have put my name on my daughters DS-230 part one, question 34 as I filled it in for her, I'm assuming that is the right thing?

I do not send ANY supplementary documents/evidence/photos (that is all provided at interview and I must show originals and have copies available right?)

I do not need to send any medical stuff back to them, I have arranged the medicals seperately and simply take the medical forms/records/photos and ourselves on the day to the medical?

Is this right do you think? Now, I am getting this ready for when I receive the packet 3 (embassy received case on 2nd March), so I only have the info I have read from here to go by. Do I definitely need to send 2 DS-156's for each of us?

I have one last tricky question. My daughters biological father has not been on the scene for over 10 years, I do not know where he lives and am not 100% certain on his date of birth (i'm only 95% certain!):

On DS-230 part 1, should I leave his DOB blank or put what I think it is?

For his address I have put 'UNKNOWN'' Is this ok?

Neither parent is listed on her original birth certificate (short form) but the full cert I have requested especially for visa interview lists us both, again no DOB. I have had to list him on the forms therefore but do not know if there is a way of proving absolutely no contact?

I have read a previous thread that told me I needed a copy of something that proves he has no legal custody order for her. I have got this but do I need a letter from a solicitor as weel to 'notorize' it? Will this be sufficient? And finally, on the IV-15 checklist where it asks for 'custody decree' do I tick it as I have a simple letter or leave it blank?

Thanks to anyone that has the time/patience to go over this for me, I really do appreciate it!

you need to sign the ds 230 and duplicate the ds 156 ds 156k ds 157 thats what i have done

  1. Mail the DS-230 Form Part I (download the PDF here) for each applicant to the Immigrant Visa Unit, 5 Upper Grosvenor Street, London W1A 2JB.
  2. Assemble all of the documents required in support of your application and mail the DS-2001 Form and check list (download the PDF here) to the Immigrant Visa Unit.
  3. The Embassy will complete all necessary administrative processing of your immigrant visa application. You may be requested to provide additional information if you have incomplete or missing documents.
  4. Approximately one month before your scheduled interview appointment with a consular officer, you will receive an appointment letter containing the date and time of your visa interview.

Bring your documents

Do not send any documents to the Embassy. You will need to bring all your original documents, or certified copies, plus one photocopy to the immigrant visa interview.

the ds 2001 looks the same as the iv15 is it the same ??

Actually this is the link for documentation for the K-1 visa, it is a relatively new page which is much better laid out than previously http://www.usembassy.org.uk/cons_new/visa/iv/iv_15.html

It is also the link given in the Packet 3 top download the forms required. If you look in the box on the right hand side of the screen titled 'More', it lists 'IV-15 and checklist' to send back and not DS-2001.
