If you are sending copies of your actual tax returns, you need to send a copy of the 1040, any additional schedules that you require, W-2. and any 1099s that were used for that year.

If you request tax transcripts from the IRS, then you only need to send the tax transcripts, and the W-2 for each year.

Good Luck!!

They do not need to be notarized - copies are ok (submit copy of tax return and all supporting documents that go with it - such as W-2's etc).

THe 1040 is the form used...the tax return is the 1040, w-2s and all supporting documents for that tax year.

By the way, if you're using official tax transcripts from the IRS, you don't need to submit with those the corresponding W-2s...

But if you are sending in your own copy of the tax return, you'll need to send in the 1040 form, W2s and any schedules, etc that accompanied your tax return.

Instructions for I-864 on USCIS website - If you provide a photocopy of your tax return(s), you must include a copy of each and every form W-2 and Form 1099that relates to your return(s). Do not include copies of these forms if you provide an IRS transcript of your return(s) rather than a photocopy.

I submitted official IRS transcripts for 2007 and 2006 without W-2s and did not receive an RFE for those items...
