ok, I will try to keep this as least confusing as possible.

My first name is spelled two different ways, with only the last letter being different (essentially the same name) one is ethnic. All the documents required by the Embassy (passport, birth certificate) and all the forms completed to date (ds230, 129, etc...) are in my birth name. On my Provincial Drivers License I go by same first name different last letter.

When I had my medical my passport was cancelled as I was picking up the new one the next day, they still noted the number, which shows one spelling, but now that I picked up my package today, I see they used the spelling of my Drivers License (which was the valid ID I provided them)

Is this a problem? I usually get my lawyers in legal matters to have me sign a document showing that I am 'one in the same'. Would this help? In the medical, they also verify your photos and also my DOB would be the same. As well, in my DS160 it asks if you go by any other names and I listed both.

Think i'll be ok? Is there an official form that I have to file for such circumstances?

I don't think it will be a problem. On the DS-156 you will be asked other names you go by. Put this on there. If there is a question show them drivers license and passport. They do not reject visas for spelling "errors", as long as they are satisfied you are one and the same, no problem

I don't think it will be a problem. On the DS-156 you will be asked other names you go by. Put this on there. If there is a question show them drivers license and passport. They do not reject visas for spelling "errors", as long as they are satisfied you are one and the same, no problem
