I-134 Sponsor and Co-Sponsor questions

i've searched and searched to find some answers but wsant able to, i have a few questions regarding the I-134.

#2 asks ......and have resided in the United States since (date) well i was born and lived in the US my whole life but, i moved to Colombia in June 2006 til July 2008, therefore do i put ....since July 2008 or my birth? (i was a resident (and citizen) of Colombia in that time)

#7 asks for my employer, the complete name doesnt fit in the selection area, we have initials which are what we always use, can i use the initials or do i have to use the complete name? for example ....UPS or United Parcel Service?

#10 asks .....I have submitted a visa petition to the USCIS on behalf of the following persons? i am doing a K1/K2 visa, do i put my fiance and her daughter or just my fiance? (im pretty sure just my fiance but just want to be 100%)

I am also going to have a co-sponsor, on my Co-Sponsor's I-134 in question...

#3..... Relationship of Sponsor? the sponsor is either going to be my best friend or cousin, what should he put? Fiance's best friend?

#10 asks .....I have submitted a visa petition to the USCIS on behalf of the following persons? he has to put NONE right?

#11 asks .....I intend or Dont intend to make specific contributions to the support of the persons? what do we put here? he isnt going to give us anything, I make sufficient money to support us but its best for me to have co-sponsor because i dont have all tax returns. Intend or Dont Intend for Co-sponsor?

Last question......Its true that i need to have 2 I-134, 1 for my fiance and one for her daughter (k2)? and the same for my co-sponsor? 1 for my fiance and 1 for her daughter?

sorry for soo many questions, we are here at the last steps to be together, i just dont want to have any problems and have everything correct, i think you all know what i mean. thank you very much in advance.

7. Attach a separate sheet. Answer should be "see attached". Make sure the attachment is clearly marked

For I-134 with children, send me an email at alshootist@yahoo,com and I will send you samples I developed from my filing. the VJ website does not iclude samples for fiancees with children for some od reason. Otherwise this is an excellent site.

11. answer "NA fiancee visa"

Yes, you need one for each. Send me email for samples. We had exactly the same situation and I can give samples made from our forms.
