The 693A no longer exists, it was replaced with an updated I-693 form that can be used for vaccinations records.

For persons between age 17 and 65

Tetanus and diphtheria toxoids

Varicella

MMR

Get them done or tested for at family doctor, or health dept, where ever it costs the least, then take record to a US Civil surgeon for transfer to an I-963.

OUR TIME LINE Please do a timeline it helps us all, thanks.

Is now a US Citizen immigration completed Jan 12, 2012.

Step 1: Go here: https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

Step 2: Call a couple of the Civil Surgeon's on the list to find which one is cheapest

Step 3: Go to the Civil Surgeon of your choice and expect to pay him/her between $125 and $200 cash

Step 4: Civil Surgeon will have the I-693 form and will tell you what vaccines you need

Step 5: Get proof (documentation) that you got all your vaccines

Step 6: Bring proof of vaccines back to the Civil Surgeon and he will take care of the rest (give you the paperwork)

My wife just had this done. We paid $170 or so to a civil surgeon. He had the I-693 and we filled out some paperwork which took about 2 minutes. Then he took her blood for an AIDS test. Then we paid him cash and left in less than 10 minutes. He gave us directions to a county clinic that gives out all the vaccines. We went there the same day and she got ALL of her vaccines. Unfortunately, she is under 27 so she needed the HPV shot. The HPV shot is only for women under 27 and it costs approximately $360 cash. It is a series of 3 shots over the course of a few months. This is what she got done: The 1st of 3 HPV shots, 1 Tetanus shot, 1 MMR (measles/mumps/rubella) shot, her AIDS test, and a TB skin test all in the same day.

Rumor has it if you search high and low you can find a civil surgeon for cheap and the HPV shot for cheap but we were in a hurry to file so we paid $170 + $360 which is $530 (I hope your wife is older than 27)!!! Also, the tetanus and MMR shots are cheap (less than $40) but since my wife is in school they gave her shots for free; and the TB skin test was free.

Hope this helps!

Don't get all those shots in one day, that is dangerous, space it out.

BTW----It is not at all dangerous to get the 2 shots in one day (Tetnis and MMR), or 3 if you are getting the HPV shot, or 4 if you count the blood being withdrawn for the AIDS test, or 5 if you have to get the TB skin test too. The nurses know every little detail about these vaccines. Any question you can think of they know the answer to. They are professionals. They give these vaccines every single day. It is no problem whatsoever to get all the vaccines at once. If it was; then it wouldn't be allowed. As far as pain: my wife is very emotional and very sensitive. She had no pain, no side effects, no complaints, not even a peep. I was surprised how well she handled it. The vaccines didn't even phase her.
