Signing forms for minors

Hi, I am just filling out the forms required for the interview and have a question. I have filled out the forms for my 11 year old, but I don't know if she signs the forms or I do? Specifically, on DS-230, under question 25 it asks for signature of applicant? Then in question 34 it asks if you were assisted in filling out the form? I have completed this part appropriately but am unsure of question 25. Does anyone know what i should do? Thanks.

a parent or guardian can routinely sign on behalf of a minor...

the second section you refer to is if you had a lawyer or the like assist in the preparation

Sign it "for them" as in Child Smith by Mother Smith or Mother Smith for Child Smith.

Facts are cheap...knowing how to use them is precious...

(child's name) by (her name), mother of minor child

good question, i had the same one as we did the DS-230 today. i told my fiance to have her daughter sign the form (shes also 11) but im going to tell her tonight to sign it and not her daughter.

one side question Kirsten UK, what did you put in 21a and 21b, is your child going to travel with you or follow to join? my step daughter is going to follow to join and we put my fiances name in 21b and NONE in 21a, since they arent going to travel together. i hope this is fine...

21a. Person named in 12 and 19 who will accompany you to the United States Now. NONE

21b Person named in 12 and 19 who will follow you to the United States at a later date. (FIANCES NAME - MOTHER OF CHILD)
