My wife is from Latin America and, therefore, has four names instead of 3;

first name, middle name and two last names, one from her father and one from her mother.

Also since we are married she is assuming my last name.

On the G-325a the applicant needs to put down their family name.

But I'm not sure which family name she should put down.

Also, on a side note, on which form should she use my last name so that it appears on her identification here in the US?

she can use your family name on any documents submitted, and in the space where it says "other names used" she can put her maiden name.

If she wishes to assume your family name, then she should put it on any visa/AOS application so that the resulting visa/green card will be in her married name.

She will also need to update her passport with her married name.

divorced - April 2010 moved back to Ontario May 2010 and surrendered green card
