Take a look at the example forms link at the top of the page. We used Claudeth's married name.

You could use your new last name for AOS application.

By the way, I just noticed that we have the same wedding date. It would be your one month anniversary this coming saturday.

i used my married name already when i filed the AOS. I had my biometric done lately so it means taht using my married name is not a problem. I also used my new signature but i also indicated my old name and signature used at the side of every page of the application forms. Perhaps it will help in order for them to keep track that i was approved for K1 visa.
