QUESTIONS ABOUT PAPER WORK

hi all well after days of trying to get everything together and submit i have 2 questions

on the g325a form at top where is says file number a do i put anything there if so what?

also on the g325a at bottom where it says This form is submitted in connection with an application for do i check the other box and type in i130 if not what sm i supposed to put there

Sorry if i seem ignorant with this i just have so much anxiety i will screw up and it will turn into horror story like some i have read on here.

Don't put anything on the file number " NONE" and the bottom yes put "I-130", good luck
