Timeline

When we went to the local civil surgeon for Tania to get the Tb skin test and have the appropriate parts of the form filled out by the civil surgeon, the civil surgeon put the forms in a sealed envelope after she completed her parts. The problem is that we had not yet fully completed section I (SSN and A# were not filled in).

Although the instructions for the I-693 indicate that it's okay to leave those two fields blank, I am a little nervous about leaving blank fields, especially when she does have a SSN I'm thinking that it might be best to take the envelope back to the civil surgeon, open it, fill in the A# and SSN, and have them put it in another sealed and stamped envelope.

I would appreciate hearing from others who have filed for AOS (successfully or still waiting) recently. Do you think we're safe sending it in as is, or is it worth our time to take it back in to the civil surgeon? Her office isn't far. Have you heard of any RFEs for this reason?

They can be left blank, I would send it in to USCIS as-is.
