New to AOS(for my wife)

Hi all,

I filed I-130 for my wife last year and she got approval notice from USCIS in Jan and moved her case to NVC(National Visa Center). I was banging my own head that I didn't check out these forums before I did that so I didn't file I-485/I-131/I-864/I-765 all together. So here we go again....

1. So I read the I-485 instruction today(2/28/09) then tried to locate these forms...guess what I can't locate I-131, I-864 and I-765 on USCIS website!!!! Does anyone know why they removed these forms? I know I can find them on other non-government websites that provide paid services etc but this is very awkward...USCIS outsourced their forms to other websites?

2. I located I-131 on visa-pro. I get confused when I filled it out name/address for both part 1: Information about you and part 2: Application type. Should I fill in my wife's name for both? What should I put under "class of admission"?

Here are the links to the forms from the USCIS website. There is a good possibility that the links have been pulled due to pending revisions of the forms. Also the I-131 form seems to have expired (top right), so you will have to wait for an updated one before submitting.
