I had my mother fill out the I-864 Affidavit of Support form for my wife because she makes alot more money then I do and I wanted to eliminate any chance of denial. In part on of the I864 what box should my Mother check:

A. I am the petitioner.

D. I am the only joint sponsor.

-or-

E. I am the first or secind of two joint sponsors.

Thank you

If she is the only joint sponsor and is only using her income then it would be D. Remember that you still have to complete a I-864 as her main sponsor even if you do not meet the 125% income level.
