Hi,

I have a few questions regarding filing I-751 form:

1. I do not have middle name, do I need to write "none" or just leave it blank?

2. For "C/O (in care of)" section, what do I need to put?

3. For "Other names used (including middle name)," I do not have any other names, on my F1 visa, the US embassy made a typo and wrote my name on the visa as "FNU Hewlett" instead of Hewlett Packard (lets say Hewlett is the first name and Packard is the last name). So, what do I need to write on the I-751 form? For my adjustment of status case (on form I-485, I-130), I wrote down "FNU Hewlett" for question of "Other names used." Do you think I need to do the same thing for I-751?

4. For place of marriage, do I need to write both city and state?

5. Does anybody have a sample on how to write affidavit of relationship letter?

6. Do you also send pictures of both of you and/or wedding pictures when you send the application?

7. When you send proofs of your marriage and living together, how many months of joint bank accounts, joint utility bills, etc you need to submit?

8. How many years of tax return do you need to submit?

9. Do we need to submit a copy of both of our passports, my previous F1 visa, copy of marriage certificate?

When you said about 3 bills per year, do I neetd to send 3 bills from right before the end of the year, or one from the beginning, one from the middle, and one from the end of the year?

What did you write for question "in care of"?

Anybody has any thought about my question #2 and #3?

In my case, I left the "In Care Of" field blank.

I became a PR in July of 2007, so we are submitting bank statements/utility bills for that month, December 2007, July 2008, December 2008 and April 2009 (we already submitted bank statements/utility bills covering the time from the day we got married till the day my AOS was approved at our AOS interview).

come down and relax the next 21 month because u have to file 751 then... not now:-)))

7/22/2010 - Online status changed to "Case sent to local office for interview schedule"

7/27/2010 - Received interview letter for 8/23/2010

8/23/2010 - Passed interview, was informed that next oath date is 9/22/2010 and oath letter will come in the mail

9/3/2010 - Received oath letter in the mail for 9/22/2010
