You may do the medical earlier than your date, as long as you have the appointment letter in hand. If you don' have it call the embassy so you can pick it up at st lukes. the number for the embassy is 0116332 528 6300 X 5184. My wifes interview is March 16 and still no packet and i called them and said they mailed it out last week

All that is needed is a copy of the noa2 or the mnl letter from the visa center and the money for the medical , just be careful as the medical is only good for 6 months , my fiance passed her interview on 2-09 and her 6 months would have been over in 10 days as we did the medical in august. got a ticket to cebu in my hand for next week and crossing my fingers on a quick delbros delivery. :thumbs:

WENDY: I'll call the embassy tmrow (monday) instead, and asked my chances to knw first what is the exact date of my medical exams, just hopefully not it reached my period days. If so then ill be requesting for early medical exams. My only worried it may fall before or after my period which is i always felt very sick a week long before and after my periods.

JOSE: then ill be attending interview date as well with ur fiancee?? dats great mentioned me to her. I'll call embassy to request to picked up my packet instead at st. lukes then. Though im just in Makati City hopefully i could get the packet this week.

JER: Ive got eveything I need original documents from my husband-to-be, I could visit st. lukes with the NOA2, I guess it wont take my 6 months validity of the visa before i get it since im just in area of Metro Manila so i will risked attending medical exams early than, prolong my interview date bcos it will passed out by medical exams reschedule bcos of my body conditions to took the exams.

I just got back from the bank (Bank of the Philippine Islands) to pay the visa fee. I brought all of my DS forms (just to be sure). Well, BPI did not accept my forms as they prefer to use the ones supplied to them by the embassy. BPI recorded my payment on the DS 157 form (Supplemental Non-Immigrant Visa Application) and not on the DS-156 as I previously posted here. They also gave me a copy of my deposit slip. Exchange rate used was US$1 = P53.00 so I paid P5,300.

Hi PW, it's me again....

I wna ask and clear these things up before i fill-up my form DS-157, it says there that:

LAST NAME ( w/c is the surname )

FIRST NAME ( w/c is the given name) - does here includes the middle name??????? like FIRST/MIDDLE

or just the given name for FIRST NAME and the middle name should be written in FULLNAME????

or LAST NAME (surname) FIRST NAME (givenname only without middle name) then..............

in the FULL NAME (should be written your whole (complete) name (FIRST-MIDDLE-SURNAME)?????

WHICH ONE IS WHICH???????????????

NEXT ..... as stated i need to fill-up forms in two copies. questions.....

DS 156 - 2 copies each applicant ( should i make one for my son as well but his name will be written

on the second page as will accompany me with the application or

travelling with me. )

DS 157 - 2 copies each applicant (should i make one of this also for my son??? )

DS 156K - well this one is for myself only.

LASTLY....... my fiance send me back form G-325A of my son, should i include these to packet envelopes???

should i just attached it in his DS 156 if im doing one form for him.

I answered "not applicable" to DS 157 item 3 because in my opinion, this question applies only to those whose names can be written in their "native alphabet" like Japanese, Chinese or Arabic characters. I just gave my Last Name and First Name. I did not include my middle name because it was not asked for.

I called the visa call center this morning and i inquire my packet condition, they said also dat it was mailed

last Jan 26th and they want me to wait till 2 weeks before interview. But i explained to "HIM" everything

and apoligized for I need to discussed it with him about my "periods condition". So he file me a request

as we're over the phone that I'll be picking up my packet @ St. Lukes on Monday Jan 20th and proceed

with my along with my son medical exams... that gives me a week before end month periods, hopefully i

won tfeel sick bcos i always do so before having it.

I also happened to inquire about the documents: they say there's only DS 156 form inside the packet

should be filled up 2 copies each of applicant.

for Manila to grant an interview after my file arrives there from NVC?

My interview date is 210 days from the time i filed. You can look at people's time lines and maybe get a better idea. I went from the Nebraska office. =D
