I got this from the US Consulate website in Brazil, FIANCEE VISA REQUIRED DOCUMENTS from the main website.

It looks reasonable. As the USC, which ones are the ones that I need to fill out? I was thinking only line #18, but I have some doubts.

Looks like only fill out the I-134.

Help fill out the DS forms also.

Plus provide as much evidence as you can of you guys having a bona fide relationship.

Good to be there if you can also.

well, yes. The I-134 is the only thing here you to provide yourself.

Remember also that any document in any laguage other than English needs to be translated and you need to give the original, a copy and the translation. So you two can decide who will provide for that. In OUR case, while I was on a visit to my fiancee, we made copies of all her documents. I brought them home, got translations done. I downloaded the DS-156 and DS-156k and completed them for her. I prepared the I-134. She went for the medical (it is one thing I could not do for her) and when I went for the interview I brought all the documents with me. She signed them as needed (DS-156K MUST be signed at the consulate) She brought all the original documents to be put together with the copies and translations and VOILA! I helped her in the process as much as I possibly could, but that is for the two of you to decide.
