Getting Immunization Records

Timeline

From my fiance :)

Dear fellow Canadian VJ members,

My doctor retired some years ago and he had all my immunizations records. Does anyone know if there is a public record of these in which I can retrieve them from?

Also, has anyone in Toronto had trouble getting an appointment with the consolate-appointed doctor? What is this doctor like (personality wise) and did you ever have issues with him/her?

I believe also the school board in which you attended should have records too, that's what my mom told me when I needed mine but I ended up getting them from my family doc.

Dr. Seiden in Toronto was really nice, no issues at all. The price just went up though which sucks.

You can contact your local health department for your immunization records.. they have them on file. I live in Mississauga and was able to get them from Peel Region. But even if your Dr. retired, did a new Dr. take over? They should still have your record or have sent it to your new Dr.

IN any case, you can just get a blood titres test done which your Dr. can do for you at no charge before your medical. My Dr. offered, but I found all my records so it wasn't necessary.

I just had my medical with Dr. Seiden's office about a month ago and had no trouble booking and the Dr. was very nice and professional. I had a female, no idea what her name was but she was nice and actually didn't do much. A nurse, who was also very nice actually did the "exam" (which is very minimal). The appointment itself was pretty much painless.

Removing Conditions

Sent package to VSC - 8/12/11

NOA1 - 8/16/11

Biometrics - 9/14/11

I had no problem at all with Dr. Seiden's office. Everyone was very nice and the medical went right along. No worrys. Just be at the office early in the morning. First in...first out and it gets very busy.

I booked my appointment 2 weeks ahead of time and had no problem- I even got to pick the day. They only schedule medicals on Mondays an Wednesday, though FYI. Also, the receptionist told me to be there at 6AM (groan!) but I think that is exceptionally early since their X-ray technician has to leave early that day.

2004-09-03 : Met at college in USA and went on first date!

2008-01-02 : Married in Canada.

2008-01-07 : DH returns to USA.

2008-04-20 : Finally received marriage certificate from the province and filed 1-130 Petition.

2008-07-08 : I-130 Approved!

2008-09-15 : Received DS-230 and I-864.

2008-10-14: Mailed DS-230 and I-864

2008-10-30: DS-230 and I-864 received by NVC

2008-11-04: RFE

2008-11-18: Overnighted missing documents to NVC

2008-11-26: Case Complete at NVC!!

2008-03-02: Medical in Toronto

2009-03-23: Interview! Approved!

2009-04-04: The big move. POE: Blue Water Bridge. All went well.

2009-04-16: SSN arrives in the mail.

2009-04-24: PR card arrives in the mail.
