is there a way to have doctor arnold do the vaccine transcription, and a different doctor, in my area, do the medical exam? like sending in two forms? would that be cheaper? not working while i am doing all this is tough!

No, that would not be cheaper. Firstly, do you need to have a full medical done? If you're sure that you do, then do not use Dr Arnold (unless you happen to live in the vicinity of her office). The fee charged by the CS that will do your medical, includes the vaccination transcription service.
