Hello VJ'ers!Its me again.I hope you wont get tired helping me out here..thanks in advance.Okay went to civil surgeon this morning and handed them our vaccination record together with the shots we had at state health department.I told them i had the full medical exam before ,after 2 hours of waiting,the nurse gave me a sealed white envelope.Is that all i need to have from them..They didnt require me to have tb skin test..

I just want to make sure if we are okay now as far as form I 693 is concern..thanks

This is what you need, do not open the envelope, simply include it in your I-485/I-765/I-131/I-864 packet when adjusting status

Is now a US Citizen immigration completed Jan 12, 2012.

Hi it cost us $130 (65 each) for me and my son.

Good to hear that you were not required for a tb skin test. Just a quick question, is the civil surgeon the one who gave you your shots. I have my medical insurance and immunization is covered by the insurance.

Can i have my immunization from our own doctor and just request a civil surgeon to transcribe it?
