Question # 20 in DS230 frm,

Do we need to list out all the address we lived from age 16 or it is just list out places/cities where lived.

My wife lived in only one city from age 16. but moved to my residence after we got married( my residence is also in same city).

Issue is, in Her PCC they listed out her old address mentioned in her passport and does not include my new address ( even tho they are in same city).

Will it be probelm?

I just listed what they asked for. City, State, and Province. No need to list specific addresses

CIty name 1992 to present

or...

City name 1992 - 2000

same city name 2000 - 2001

same city name 2001 - 2003

same city name 2003 - 2005

same city name 2005 - present

I simply put "City name 1992 to present" and had no problems. No need to list separate addresses in the same city.

Just do not forget to fill-out new DS-230 (Application for Immigrant Visa and Alien Registration) form available at http://www.state.gov/documents/organization/81807.pdf with new expiration date of 02/29/2012.

I do not know, if they accept old form or not.

The new form was just released Feb of 2009. they can't expect the new one to be used in the same month it was first issued. If you are using the new one great, if using the old one don't worry they should give a 30 day grace period minimum considering mailing from foreign countries Ect. If you are overly concerned just call NVC and ask an operator. When i call today I'll ask that question and post a response when i get the info.

OK regarding the new DS-230, I just spoke to a supervisor at NVC and she told me the new form is for preparing them to go to electronic system and that any "old form" will be honored for the time being. "no packages will be sent back due to the old forms". That was from an NVC agent not just an operator so no need to worry if yours is in the mail.
