You can find the Alien Registration # in the NOA-2 of your K-1, it is usually under the Beneficiary box. We didn't enter anything for the 'File Number'

A# (Alien Registration number) can be found on:

  • NOA2/Approval Notice
  • Visa

Her maiden name is her maiden name - she didn't change her maiden name, (presumably) she changed her last name (aka married name).

You can also find the Alien number on the I-94 stapled in the beneficiaries passport. It will be the hand written number that starts with an A.

Yes, you need to put your wife's maiden name.

Please refer to the example forms: http://www.visajourney.com/examples/INS-Form-G-325A-aos.pdf
