Hi All,

am preparing forms for my AOS.i`ve some questions i couldnt solve.

1. Part One it asks:A#(if any) , what is that ? my NOA2 A#number that has 9 digit ?

2-Date of last arrival? is that the date i entered the POE, it is stamped on the I-94.

3-I-94#: is that the number on the top left of my i-94 , it has 11 digits?

4- Part II: it asks : Nonimmigrant visa number? is that the red 8 digit number written on the bottom of my visa?

Last question: the forms i need for my AOS are , I485.I-864.G-325A.I-765.AND -131 ?

1. My wife's A# was written on the back of her I-94

2. Date of last arrival is the date in which you entered the United States by whatever port you came through. My wife entered through Halifax which has US preclearance.

1. A number is most reliably found on the NOA2, the 9 digit number with an A. Some POE's don't write the A Number on the I94, some embassy's don't write it on the actual visa either. But its always on the NOA2.

22nd September 2008 - NOA1 x 3 Received

6th October 2008 - Transferred to CSC

17th October 2008 - Biometrics

5th December 2008 - EAD & AP approved

20th February 2008 - GC approved

I-751

================

1st December 2010 - I-751 Sent (CSC)

3rd December 2010 - I-751 Received

7th December 2010 - Cheque cashed

3rd January 2011 - Biometrics

Don't forget the I-693. Or just vactination transfer by civil surgeon if you already had your medical.

hi, thanks for your answers, but what is the i-693 ? i had my medical exams done before i came here and i have a copy of form DS-3025 and i think i gave it with the sealed envelop at the POE. Do i need others vaccinations or tests? thanks in advance.

8. All applicants required to submit I-693, Medical Examination, are also required to submit a Supplemental Form I-693, Immunization Record. However, applicants who last entered the United States as a K nonimmigrant (I-94 Arrival/Departure Record and/or the nonimmigrant visa page in passport shows K-1, K-2, K-3, or K-4 as admission code) are usually required to submit only the Supplemental Form I-693 (immunization record) with the Form I-485.

9. The I-693, Medical Examination, and I-864 should include any supporting evidence requested by the instructions. Do not have the civil surgeon mail the I-693 to the NBC. If responding to a request for evidence, the I-693 must be submitted with the request for evidence letter so the response is matched to the proper file. Do not submit chest x-ray film as part of your medical examination.

10. The I-693, Medical Examination, and/or Supplemental I-693, Immunization Record, must be submitted unopened and in the sealed envelope from the civil surgeon. The civil surgeon should provide a copy of these forms to the applicant before sealing them in the envelope that is submitted with the I-485 application.




It's from USCIS website. You can find civil surgeons on USCIS website closest to your location. If you can find a copy of DS-3025, just ask the civil surgeon to transfer everything on Supplemental form of I-693(it should cost less than the whole medical exam) If you dont enclude this form now, you will get a RFE later or during your interview.
