Where do I address packet #3 to and new letter of intent to marry?

Hi, in the top left of my letters, I'm not sure what to put as far as an address goes for the cover letter for packet #3 and new letter of intent to marry?

Also which originals do I need to send to my fiance for her interview?

-I have all of our originals from first package sent for NOA1 including all original receipts, photos, emails, snail mail, itinerarys, etc.

-Original letter from my bank stating $$$$$ in my checking account

-Original letter from my boss stating my annual salary, start date, and my full-time status on company letterhead

-Tax transcripts from 2005, 2006, 2007, and my new 2008 tax returns

-W2's from 2005, 2006, 2007, and 2008

-My original birth certificate

-New Original letter of intent to marry

-Color copies of my passport

I was born in the US and am a US citizen. My parents were born here in US also. Should I answer question #2 on the I134 which states "I am ___ years old and have resided in the United States since (date) _____. I'm 29 and have lived in US since birth?

Address the intent to marry letter to the Embassy. Can use the example letter in the example forms http://www.visajourney.com/forums/index.ph...p;page=examples

Note Manila combines P3 and P4 into one packet everything is brought to the interview, so no cover letter required.

Where should I address the supplemental information sheet if her address does not fit? I'm going to have to type "See attached page"?

Either hand write in address, or "See attached" And on the attached make NOTE what form the attached sheet is for.

Originals, should be brought to interview, in case they question the validity of the copies in the case file.
