ds-157 question #12 (for those who have worked in JAPAN

im bringing up my query again... im just confuse about filling up the DS-157 form #12 regarding employer's info. Should i write the agency name here in the Philippine where we process all our papers or the employer in Japan? how to fill up the name, address,phone number and supervisor then? The promotion/agency's name in the Philippines or the employer in Japan?

Hi there!

I used to work in Japan too. We filed ours last November 2008. In my case I put my Japanese Employers name for consistency(with the other info like address) but then you have to make sure that this company will give you an employement certificate for future use too.
