How to read/input philippine address

Hi,

I noticed on the visa forms it ask for the name and address of your fiancee or spouse and it list street, city , state, etc. just like in U.S. address format. How would you read and input a philippine address into this form format. Here is an example address:

KM8 PUROK 12-B TABON MANGAGOY BISLIG CITY

What would be the house number, street, city State etc.? Basically, how does this relate to our address format in the U.S..

Claudeth's was weird. It was zone 5 in bayabas and then her family's compound. It actually states her family name like Smith Compound. There is also some sort of zip like thingy, 9000 was what we used.

Hi it should be like this...

house number KM8 PUROK 12

street TABON MANGAGOY

city BISLIG CITY

state (is her province)

ask her what province she is in right now to put for the state and also their zipcode
